What would cause a new alternator belt to simply disappear?

Not having it installed correctly and driving the vehicle. If not installed correctly, the belt may have been out of the grooves a little and slipped completely off while driving, resulting in loss of belt. Or you may not have had it tensioned tight enough and then it slipped off. Either way, faulty installation is what I would suspect.
