It won't shift into any of the gears until you reach 4000 rpms

It could be a shift solenoid, low fluid, control module. Has the check engine light come on?

Just to add to this one if you let up on the gas a little will it shift to the next higher gear?

Usually when you have to let up on the gas for the transmission to shift on that car its due to a plugged cat. Take it to a auto parts store and have it scanned for codes post the exact code numbers not the code descriptions.

I have a 2002 L200 that does exactly that. Shifts into 2nd at high rpm until I let off the gas a little. It happens only on initial start in any weather. After warmed up, it seems to shift better. You mentioned a plugged cat might be the issue. What is the cat?
