CAN BREAK A VALVE AND DRILL THE ENGINE CYLINDER 2JZ GE BEING IN POWER WITHOUT ACCELERATING IN PARK?

My car had a fault in the coils and the mechanic told me that at the end of change that when doing the tests having the engine running without speeding it broke a valve and the cylinder is drilled.
why did this happen?

Sometimes stuff happens and does this. It may have had a piece come out while doing testing.
