Toyota Rav4 Repair Question

RAV4 clunking noise in rear with shifting
I have a '96 Toyota RAV4 with 70,000 miles that produces a clunking noise when I manually shift into 5th gear. I have also noticed a soft grinding noise when I am moving in reverse. What are the possible problems associated with these noises?
