1993 Toyota Pickup Timing gear

Engine Mechanical problem
1993 Toyota Pickup 4 cyl Two Wheel Drive Manual 145250 miles

When replacing the timing chain and gears do you put the dot on the cam gear at 12 o'clock or
the notch that fits on the cam dowel at 12 o'clock ? The manual says the notch but it runs rough and cannot get the timing to set at 5*.
