92 Camry has trouble starting would like HELP!

I have a 92 camry 4 cylinder 191,000miles. In the morning when I go to start the car it doesn't start, I would have to put the car in nuetral to start the car and step on the gas it would turn on for a second then shut pack off I would then do it agian and it would do the same, and again then it would finally start, I would like some help on figuring out what I need to do? If it helps any there is also a fair amount of smoke coming out of the muffler, a lilttle more than usual, I also had a tune up 2 days before a few friends say a new fuel cap is needed because the car was seating a lilttle before I brought it
