Engine revs up between gears

2001 SEAT Alhambra 4 cyl Two Wheel Drive Manual 61000 miles
I have a 2001 seat Alhambra which when you change gear the revs jump, most noticeable around 3000 revs they will then jump to 3500-4000. If you change gear very slowly it's ok but if your trying to pull out from a junction etc it's terrible. A few other things I've noted are there seems to be a clicking noise like an actuator which will carry on after the engine has been switched off, also it only does about 11mpg and lastly there seems to be a vacuum leak on the brake as every now and then there is a hissing by the pedal. I've taken it to several garages but the diagnostic machines will not connect for some reason, but there is no light on either.
