Transmission problem

I have a 1995 Plymouth Grand Voyager 125,000 miles
I have 2 questions: #1 When we drive the van for sometime and turn the engine off and restart the engine the transmission will default to 1st gear. We turn off engine & let sit for about 15-20 minutes & restart and it will act ok. When it has not "cooled" off enough it will go into drive w/ a hard clunk. If we never turn engine off and leave it in drive it will act normal. Also the hotter the weather the quicker it will show up whereas the cooler the weather the longer it takes to show up.
I've checked fluid level its ok.
#2 Every once in awhile the clock, radio, & tape player will not come on when van is started but will come on sometime later when a different time. It may stay off for days and it may act normal for weeks.
The FM has quit a few years back if this helps

I would check your relays to see if that might be the problem if you say the fluid levels are ok. You might have a bad relay. You can find them under the hood in Your PCD right next to the battery.
