1987 Jeep YJ transmission problems i think

1987 Jeep YJ 4 cyl Four Wheel Drive Manual 136000 miles
i have multiple problems that i think are all related to the transmission but am not sure. sometimes when switching gears, usually between 3rd and 4th, the engine revs when the peddle is not pressed. there is also a clunking sound after the jeep runs for over 10 minutes, near wear the transmission is. also just recently after driving over two hours it became very difficult to put the vehicle into any gear. is this last problem due to the transmission, or is it more likely involving the clutch.
