1999 Isuzu Rodeo Question Clutch pilot bearing

Drive Train Axles Bearings problem
1999 Isuzu Rodeo Question 6 cyl Two Wheel Drive Manual 135000 miles

Clutch went to the floor and would not come back up on its own, when I pulled it up with my foot it would not depress again, Pilot bearing was changed, and went about 40 miles and the same thing happened again replaced pilot bearing again and on about the 5th shift pedal went back to the floor again. Mechanice told me that the pilot bearing is pulling out.
Is there a adjustment that we are missing?
