2002 Honda CRV Car jerky going into third gear and now stal

A couple of weeks ago, my CRV started jerking as it tried to go into third gear. If I let off the accelerator and then pressed the accelerator again, it would go right into third. It was low on transmission fluid, so I topped it off. Prior to checking the fluid, it was happening almost every time I drove it. Now, it happens only 1/4 of the time. I don't know if this is related or not, but yesterday it worked perfectly and then all of a sudden died while I was stopped at a stoplight. I make it home, but only by keeping constant pressure on the accelerator. If I let off the gas at a stop or trying to drive/reverse out of "park", it dies.

I am having a similar issue with my 2000 Honda CRV (128k miles). It has the issue where it gets stuck in the lower gear around 1500 rpms, but also dies when I try to switch it out of park if I have not let it warm up for 10 minutes. I have visited two Honda dealers for help and received no assistance. If anyone can help that would be great because I expected this car to live for 200k miles and right now it is not looking like it will!
