goes off on motion

i realise that when i am driving,the car just suddenly goes off on motion.when this happens,i will try to start the car again and it takes a while before it starts.even after starting,i realise that when i put the gear on reverse ,it jerks heavily and when i put it on drive,it is sluggish.so i have to switch off the ignition of the car and have to wait for a couple of minutes and then start the car again ,then it works normally.this occurs like 3 times a day continously before the car now works normally.i scanned it and all i got is P0726.Please i need some professional advice on what to do.
