1995 Ford F350 slipping

In fourth and fifth gear the rpm's rev up, and the truck stops pulling when I hit about 1500 rpm's

You have a faulty clutch i'm afraid. It needs replacing. I suspect it's the pressure plate and it will only get worse?

Slipping
Pressure springs worn or Release pressure plate
Oily, greasy or worn facings. Clean or replace clutch disc
Incorrect clutch alignment. Realign clutch assembly
Warped clutch disc or pressure plate. Replace damaged components
Binding release levers or clutch pedal. Lubricate and/or replace release components
