Head gasket or cracked head?

99 Ford Taurus, 150000 miles. Overheating, loss of coolant, already replaced radiator, water pump, thermostat, and heater core. Coolant spraying from the radiator cap from pressure. Is it a cracked head or head gasket?

You won't know for sure until it is apart, and then still may not know. ALWAYS have a machine shop pressure test, clean and shave heads or might end up back in the soup.
