2000 Ford Explorer Right Rear Spark Plug Replacement

At 100,000 miles the owners manual says to replace all the spark plugs. I've replaced all but the right rear plug. I can barely get to the wire from the top or underneath. From underneath it seems like I'd have to remove the exhaust manifold, and from the top the entire heater. How does one get to this plug to replace it? Which items have to be removed?
