1992 Ford E-Series Van key will not turn in the ignition

1992 Ford E-Series Van V8 Two Wheel Drive Automatic 310000 miles
i have replaced the ignition lock and key when i did that by drilling it out the new one worked for a week now i am having the same problem again. this time i took out the ign switch key still wornt turn. do you think it could be the steering wheel locking mechanism or the shift lock mechanism?
