Skip the first question. The nut spun off the yoke which allowed it to pull into the casing and nearly every component was wrecked. Mostly worn parts with that many miles and neglect caused the catastophic failure. The pinion shop had another truck identical to it sitting there with the same problem. CHECK THE DIFF!
