Cannot control air flow

We are not able to control from which location (floor, dash, windshield) the heat and A/C air flows. We can adjust the temperature and the fan speed, but we cannot get the heat to the floor or the A/C from the dash.

The typical issue with this is the mode door actuator. This is the actuator that controls where the air comes out.

I am also attaching the info on this from the manual.

Please run through this and let me know what you find with this as this is what I would replace if it were me.
