my caravan tripped the following coeds 045.6 and 168.4 what is the definition of the codes i cant find out anywere

How did you read codes like this? If these came up with an OBDII scanner, your PCM is giving erroneous codes and might be bad...codes begin with letters...P is the normally scanned code series for sensors related to emmisions. C for ABS, and U for body codes. Fill me in! What shows these numbers?

P0456= Evap small leak detected.
