Doesn't continuously blow warm

2002 Chrysler Grand Voyager 6 cyl Front Wheel Drive Automatic 110000 miles
Recently our minivan blows cool air when the heat is on after the car is sufficiently warmed up. It will blow warm every once in a while but will cool off shortly. Mostly blows cool. Fan and resistor were replaced a few months ago and seem to be working fine. What makes it go to cool air when on heat? Thanks

Could be a thermostat, air mix/blend/mode/recirc door problems, vacuum and actuators problem or the HVAC control module
