2000 Chevy Impala 102000 miles

My Chevy Impala starts to jerk back and forth when I accelerate at low speeds. What causes this?

I have already changed cam sensor. My codes came up as: PO341, P0420, PO742. I also changed my spark plugs and wires. No results. . . I also gave it a radiator flush. Last but not least coolant temp. sensor. . No results.

I forgot to mention that lights come on periodically like: check engine, service vehicle, low ccoolant level, change oil. However my oil looks good and there is proper coolant level.
