2005 Mazda 3 cooling/ heating system

My radiator was changed. Took it home. Next day driving my car temperature gauge turned from normal to Hot to cold and continued to do that. Engine light come up and heating ( cabin)system stopped working. Called dealer back. They picked the car. Found the lower hose was not tight. All coolant had come out. Fixed(?). Now. My cabing /heating system still not working consistently. Dealer still has the car. ! Week later. They say they have not found the problem. Changed the thermostat and car still cools down when low revs.
Questions:
. What could be the problem?
2. Could this be caused by the engine overheating?
3 How do I know if the car suffered permanent damage?

For overheating problems, damages would not stay for long. If the head gaskets are bad, the overheating would come back with possibility of oil getting to the engine oil or vice versa.

Get a second opinion if necessary and there are chemical test available to determine if the head gasket is bad.
