2004 Toyota Tacoma Power loss, hesitation, stall

At 1500-1750 rpm it seems that my tacoma stalls/power loss after about 20-30min of driving. And also at cruising speed 70 mph. The stall/power loss last about 3-5 sec. And not all the time. It also seems it happens at stop lights when I'm taking off. I had the TB cleaned along with the map sensor and it still has the problem. Could it be the TPS? It's getting dangerousness especially at stop lights.

Has the fuel filter ever been replaced? Have you checked fuel pump pressure?

Not sure about fuel filter or pump pressure. I bought the truck about a year and a half ago. It didn't start happening until a few months back
