How do I find out if I have a rear heater core for my 2003 town and country?

Well, if it has rear controls, it has a rear heater.

How would I find the heater core for it? At the moment I know I have a core leak in the front. It is leaking on the passenger side. Is the front heater core only one in the front?

Yes, there is only one in the front. The rear one is in the side panel at the rear.
