What could be the issue I'm having with my car jerking when switching gears?

My car, once warmed up, jerks when switching from 1st gear to 2nd, and then from 2nd to 3rd. If you give it some gas, the jerking will cease until the car switches gears again. What could be the issue. Keep in mind, it's only when the car has been driven for a while & is warmed up. The jerking is definitely noticeable and of heavy nature. It was mentioned that the issue could be the shift solenoid.

You could be right on the shift solenoids or its actually hesitating due to the fuel pressure, MAF and TPS sensors
