How to diagnose transmission problems.

For the last 3 years i have had no first gear in my truck. when in first gear it just acts like the truck is in neutral. i have put on about 40k miles on it with this problem. Everyone i ask keeps pointing me in different directions but none have been helpful. What could the problem be. I am not a tranny guy at all, so any help would be great.

If you no 1st, have the computer scanned to identify if there are transmission related trouble codes.
