Ford F-350 Dent Problem

The truck looses power whenever I'm pulling a trailler or weighted up hill, what is causing this problem? How do I DIY fix it?

Cat could be getting plugged up. Look at the exhaust back pressure when condition is evident. 300k might have the motor getting a bit tired as well.
