Plymouth HVAC problem

1998 Plymouth Expresso 3.3L. I have run all diagnostics on HVAC system and get error code 1-4: "Blend Pass. Actuator did not reach heat stop" error. I need to inspect/replace the actuator, i think and/or inspect/fix a stuck door, i think. I have removed a majority of the dash but i don't know where to find the actuator or get access to the door. Can you please tell me where and how to access these so I can ge the HVAC system working again? thanks, Ken

I'm not sure what an Expresso is but this info is for a Voyager.
It may or may not be the actuator itself causing the problem.
