Ford E-Series Van Engine Problem

95 Ford E150 - 8 Cylinder/5.0 Liter, Van runs great when cold but will hesitate, misfire and run poorly after engine is run for a long time under heavy or stop and go conditions. Also, when engine is cold, then driven on short trips, it will run great until turning engine off. Then run poorly after restarting shortly. So short trips running errands around town is problematic. If engine is given an hour to cool down, it will run great again.

Sometimes, engine will run great if I turn ignition key to on, then off and on again three times, then wait 60 seconds until starting engine. This works 30% of the time. I'm not sure if this is resetting computer and sensors. I am always amazed when this works.
