Toyota Corolla Engine Problem

Failed the smog registered gross polluter idle keep going up and down some leak starts and run fine. Do I need to overhaul or replace engine?

That would depend on what is wrong. Emission problems are usually due to faults such as fuel, ignition or exhaust system. It could even be due to vacuum leaks so you need to have the problem diagnosed to find out what needs to be done.
