Transmission problem

Hello, I have a concern about my Lincoln. Once a week or more the check track light comes on. It happens when I push on the gas, the car will jerk and sometimes it will completely die on me. Most of the time it jerks and then the light comes on, also happens when I go up a slight incline. I have had my transmission and radiator flushed and it still does it. Most of the time the car runs like a champ, but just today the track light came on. If you have any suggestions that we can do I would greatly appreciate it. If it is easy enough to do without taking to a car shop that would be even better. Thanks for your time.
