Have your injectors checked for signs of leaking. Do a static pressure leak test with a fuel pressure gauge. Just turn the key on or shut the vehicle off, and the pressure should slowly drop down on the gauge, if it drops down real quick you know you have a leak somewhere. You can start disabling injectors, pulling plugs or other forms of diagnostic to figure out which injector it is.
There are a few other possibilities but this seems to be the most common that I've seen atleast. Do you have any codes?
