Engine Performance problem

Hey Guys, I have a 1998 Ford Contour SVT, with appr. 150,000 miles. I have a somewhat unusual problem!? When i am driving no matter what speed, when i shift up or down as soon as a step on the clutch my RPM's will increase dramatically and if i keep the clutch in they will very very slowly come down. i noticed if i feather the clutch it will drop faster but am stumped. I have replaced the TPS sensor, Fuel filter, O2 sensors, EGR valve/solenoid. Sometimes my ECM throws a generic code that describes anywhere from a vacuum leak to a whole bunch of misc. things but then goes out all of a sudden. I did notice that if i clean the throttle body really well it seems to go away for a little bit but then will ret(about 6 months) anyhting you might be able to rec. would help!! sorry so long thanks guys.
