Transmission problem

transmission has been replaced once already and now it is ready again?

early symptoms; lag when changing between gears, momentary surge when stoping.

now; goes fwd but not more then 40 mph. if stopped on an incline in will not go fwd. will not go backwards but will go into reverse. have it on a car dolly and to get it off the dolly have to put it in reverse and push it. it will not go backwards in neutral.
