Engine Performance problem

First off I have a 96 probe 4 cylinder, it idles good, but when you give it some throttle it dies down a little then revs up fine, while riding down the road at steady throttle car will begin to die out until I give some more gas then it will run fine. At about 3,400 rpm at 3/4 to full throttle it will act like I let off the gas and go back and forth jerking, half throttle it will run fine past that spot until I give it more gas and it acts like it is flooding out or not getting any gas. I have the trouble codes and they are P0171 which says system lean bank 1 and P1130 which says manufacturing controlled air and fuel metering. I have changed the distributor cap, rotor button, upstream O2 sensor, throttle position sensor, ignition coil, and ran fuel injector cleaner several times. Voltage on mass air flow sensor is fine. Please give me some suggestions. Thanks.
