If there are 5 of those relays in the pack, my info shows the middle one is for coolant fan.
Just to be sure, you can look at the wiring colors on the diagram for that relay, match-up the colors with the colors on your connector.

Did you try taking the electrical connector loose from the fan, use jumpers to run B+ and ground from the battery? Just to see if the fan will run? You might use an inline fuse, just incase.
Sometimes those relays in the relay pack are the same, you might be able to swap one of the others for the suspect relay, just for testing. My info shows the outside relay on the passenger side is the ac compressor clutch relay, you might be able to swap it out, if their the same, with the coolant fan relay, for testing. Again you might look at the wiring diagrams to see if the colors match up on the connector. If the ac compressor clutch will kick in, after the swap, then the relay maybe ok. I'm assuming your ac system is in working order?
