Transmission problem
1990 Acura Integra 4 cyl Two Wheel Drive Manual 186000 miles

I have slight grind on 3rd gear and when i put in 5th arter a few miles it pops out , I whant to know what can be causing this problem or if I need a new tranny.

shift fork is probably bent, it usually happens after someone has been either slaming or leaning into the shift lever for some time,either way, its due to excessive force on the lever.
you can get it repaired instead of spending for a new transmission. it's not considered major as far a transmission problems go
